Aberdeen City Health and Social Care Partnership (formerly known as Aberdeen City Community Health Partnership) manages a wide range of community based health services and, in partnership with local councils, provides a focus for the integration between primary care, specialist services and with social care to ensure that local population health improvement is placed at the heart of service planning and delivery. The Moray Community Health and Social Care Partnership is the organisation which brings together Dr Gray’s Hospital, community care services at the Moray Council, public health services, primary care, mental health, learning disability, health improvement and community health services.
