Below is the documentation to support the Aberdeenshire Integrated Assessment Framework. Guidance on using the documentation for Assessment, Contact Record, Core Record, Chronology and Action Plan will be found in the Operational Guidance below.
Please refer to the IAF Process Flowchart to help you confirm that you are following all the steps in the process whether you are applying it to your team (the yellow stage), your Service (the blue stage) or at a multi-agency level (the green stage).
Two sets of exemplars for the IAF process have been developed to demonstrate how the templates (for Contact Record, Chronology and Multi-Agency Action Plan Minute and Action Plan) should be completed. There is one for a Secondary age young person and one for a Primary age child. Each exemplar contains a story / timeline for the young person and then the relevant completed paperwork. These young people and their stories are fictitious and have been created to support the use of the IAF templates.
The 'Well-being Indicators' are the basic requirements all children and young people need to reach their full potential. We can use them to help us identify those children who may not achieve their potential because some or any of these indicators are not being met. The 'My World Triangle' helps us carry out an assessment of all the strengths, needs and challenges within the child's life so we can understand why the requirements are not being met. Each Service has developed prompt questions for the My World Triangle headings. You do not need to ask every question if they aren't appropriate - assessments should be relevant and proportionate.
You should only use these templates if your Service or Organisation doesn't have any system (paper or electronic) for recording this information.
