Does each service have to have a signed Multi-Agency consent form?

No;  where several agencies are working with a child / young person and their family, the lead professional will have a signed Multi-Agency Consent form and this covers every agency / service that is involved - as long as the person obtaining Consent has explained which agencies / services will be sharing information. If an additional agency / service joins the team around the child to provide services, the lead professional or most appropriate person would need to discuss this change with the child / young person and their family and obtain signatures for a new Multi-Agency Consent form.


Can attendance at the Awareness Raising Events be included in my Continuous Professional Development Log ?

Yes and we will be sending out 'Certificates of Attendance' to support delegates in maintaining their CPD log.


Do I have to draw up an IAF Action Plan as well as  the relevant Service Plan that I have already prepared e.g. if we are producing a Children and Young Person?s Plan or Co-ordinated Support Plan, does the child also need an IAF Action Plan?

No.  The child / young person only needs one plan which can be a CYP Plan or CSP.  What is important is that the plan that you have prepared for the child / young person includes all the elements of the IAF process and IAF Action Plan and they and their families are actively involved in the assessment and planning process.


How do you identify who should be the Lead Professional?

The Lead Professional should be chosen in discussion with the parents / carers and child and should be the most appropriate person to take on this role at the point of drawing up the Action Plan.  The Lead Professional will not necessarily be the person who initially identified the need or arranged the first Action Planning Meeting.   The person who takes on the role of Lead Professional may also change over time as the needs of the child / young person change.
